- More than 24 million people in the US are currently diagnosed with diabetes. An estimated 6 million – twice the city of Chicago’s population - remain undiagnosed. Over 80 million (nearly 1/3rd of the US population) are pre diabetic who are at risk of developing diabetes if they do not adopt life style modifications.
- In the United States nearly 45 million people suffer from high blood pressure. That is nearly 1in 4 adults. Only half of them are treated due to lack of proper diagnosis. High blood pressure is a leading cause of stroke, heart attack and kidney failure

- The incidence of periodontal disease manifesting thru bad breadth, bleeding gums and eventually loss of teeth is no longer recognized as simply a threat to oral health, but has impact on heart health, kidney disease, diabetes and osteoporosis.
- Every 45 seconds, someone in the United States has a stroke. If blood flow is stopped to the brain for longer than a few seconds, brain cells can die, causing permanent damage and the tragedy of life long disability.
These are sobering realities we face in the racy world of computers, commutes and fast foods. While some of these realities may be genetically unalterable, we at CCN strongly believe that many can be altered through life style modifications to improve outcomes. Hence our emphasis on health education and health screenings to promote health awareness within the community.
CCN’s additional mission to provide affordable access to health care for the uninsured is no less challenging. With over half a million uninsured in Chicago and suburbs, this issue has become over whelming for both poor and not so poor. Our focus has been in providing a medical home within our network for the care of chronic ailments, the bulk of which are responsible for the biggest share of health care costs within the nation. Conditions like diabetes, hypertension, coronary artery disease and obesity.
This year CCN has started a pilot program to help individuals with weight problems. CCN’s Health Coaches are getting trained through Weight Management Workshops, to become personal coaches who will educate and encourage interested participants, on life style modifications involving healthier eating and regular exercise routines.
All this is possible because of the services and sacrifices of the compassionate few who see the vision and are willing to share the burden of the people they serve. Of course without the generous financial support of a few more we could not exist. To them all we owe a debt of gratitude. May Allah bless them and reward them.
