Compassionate Care Network (CCN) is a program for the uninsured in the Chicagoland area. CCN currently has over 100 participating physicians, specialists, dentists, optometrists, chiropractors, podiatrists and physical therapists. Several imaging centers, full service pathology labs, and home health agencies are included in the network and provide services at discounted fees.

How Do I Join CCN?

1. Complete an application form for enrollment. Each person to be covered must fill out a separate form. Forms can be downloaded on the homepage.

2. Choose a primary care physician (PCP) from within the network.

3. Pay enrollment fee. Please make checks payable to CCN.

4. Mail your application with payment to:

5. You will receive you CCN ID Card by mail in 1-2 weeks.

Individual - 6 months - $60

Family - 6 months - $90

Individual - 1 year - $120

Family - 1 year - $180

Do I qualify for CCN?

Yes you do if:

1. Your annual household income is less than 400% of the Federal Poverty Guidelines as show below:

2. You have no health insurance for the services CCN is providing.

3. You live in Illinos.
